моя тактика была простая: как написано в документах - так и писал в формах. Дата обретения гражданства РФ у всех рождённых в СССР одинаковая, т.к. новый закон о гражданстве приняли в 200x годах, но чем это объяснять офицеру лучше поставить наиболее простой ответ - дату рождения. В офоциальных документах эта дата вообще нигде не фигурирует. У детей проще - у них штамп на свидетельстве о рождении есть "принят в гражданство РФ такого-то числа".

В загране стоит USSR, везде так и писал, проблем не было.

У меня в загране указано место рождения Азербайджан, а в свидетельстве о рождении - Азербайджанская ССР. Плюс еще город, в котором я родилась, в 1991 году переименовали. Может, попросить нотариуса приписать, что страна и город были переименованы после развала СССР?

Для страны выдачи birth certificate пишу USSR, вот только при записи на медицину там такого пункта нет. Зато есть Soviet Union! Его и пишу.
